About the Role: As a Medical Assistant - Healthguide, you will work closely with clinical teams to strengthen the connection between patients and their primary care providers. You will play a crucial role in improving patient health and well-being by helping them navigate their care and enhancing communication with their medical practice.
Key Responsibilities:
Collaborate with the patient's medical practice staff and our Case Management team to manage a targeted patient population.
Work independently while also being an integral part of a collaborative team.
Assist patients in achieving better health outcomes by addressing their questions and concerns, connecting them with the medical practice, and implementing effective care plans.
Accurately document patients' medical history and issues in the electronic health record (EHR).
Understand and assist with the referral and prior authorization processes.
Build trusting, professional relationships with assigned medical practices and within our organization.
Meet assigned Key Performance Indicators (KPIs) to ensure positive patient outcomes while minimizing medical spending.
Utilize various communication methods (calls, texts, portals, emails) to engage high-risk patients.
Escalate medical issues appropriately to the Health Guide Registered Nurse, Primary Care Physician, or leadership.
Travel occasionally to supported primary care offices.
Adhere to all security policies and procedures to protect Protected Health Information (PHI).
Qualifications:
Certified or Registered Medical Assistant.
Bilingual (English and Pashto) required.
Passion for patient coordination, especially for individuals with complex medical and social needs.
Empathy and a willingness to learn motivational interviewing, critical listening, and behavioral change management skills.
Strong conversation skills and the ability to document patient interactions accurately and in real-time.
Knowledge of medical terminology and primary care practice functions.
Familiarity with social determinants of health.
Certification as an Emergency Medical Technician is preferred.
Proficiency in using electronic health records (EHRs) and care management analytics databases.
Ability to manage competing priorities while maintaining attention to detail.
Experience with various EHR systems such as EPIC, Athena, or Cerner.
Exceptional written and verbal communication skills.
Ability to track multiple tasks effectively while ensuring timely completion.
Demonstrated success in a remote work environment.

Compensation:
The base pay range for this role is between $20 to $23 per hour, paid bi-weekly. Final compensation will be based on various factors, including skills, experience, education, location, and certifications.
